Retreat Application Form

The total cost for the Weekend is $150.00, which includes meals and lodging.  If you are interested, please mail the application form and a $60.00 non-refundable registration fee to the address above.  The remaining balance is payable at check-in.  Participation is limited by space availability, so early registration is encouraged.  NOTE: If you are in therapy, you must provide a written consent from your therapist to attend the retreat no later than the time of registration.
